Around 1990, my Dad was doing some work on "human factors" related to a manned Mars mission. Medical issues are a big thing, but he was especially concerned about psychological issues.

For example, one of the big recreational activities on Earth-orbit missions is just looking out the window at the Earth. Can't do that on a Mars trip -- for most of the flight, the view consists of a tiny blue dot and a tiny red dot.
